Living Well @ Home was established in 1993 as an affiliate of Kisco Senior Living as a licensed home care agency. Our team is ready to answer any questions you have and guide you through the variety of services available.
COMMUNITY TRIAGE PROGRAM
Our Triage program consists of in the moment support and guidance if health concerns arise. This program is a provided at no additional cost to all Independent Living residents at Abbotswood at Irving Park.
PERSONALIZED SUPPORTIVE LIVING
When and if more care is required, we are here for both short and long-term needs. Allow the caring support team at Abbotswood to help you navigate your care path, to maintain optimal independence, and quality of life. Care plans are guided by a personalized care assessment. Our nurse manager will partner with you to match services to your individual needs.
ON-SITE DOCTOR VISITS
Don't want to visit the doctor? Then let the doctor come to you! Abbotswood at Irving park offers on-site visits from different local providers. Ask our team for the monthly calendar to view the regularly scheduled dates (contact the provider directly for all appointments).
